Insurance and Payment Options

At Commonwealth Eye Care Associates, we believe that each of our patients from Richmond, VA deserves the highest level of personalized eye and vision care, regardless of how much they earn. We understand that some of the services we provide are not covered by insurance, and that our patients may incur some out-of-pocket costs for treatment. To help our patients fit care into their budgets, we offer multiple payment and financing options.

General Information

It is our policy to collect any co-payments at the time of your appointment. We accept payment via cash, check, credit card, and money order.

We offer online bill pay to make our patients’ experience with us as convenient as possible.

Insurance Accepted

Medical insurance providers often require pre-authorization for eye and vision care services. Before your appointment with us, please discuss any pre-authorization requirements with your insurance company.

Commonwealth Eye Care Associates participates with most major insurance carriers, Medicare and Medicaid. For particular questions regarding your insurance or to see if we participate with your carrier, please call 804-290-4278 to speak to one of our insurance specialists.

Disclaimer

We will check your benefit eligibility and make every attempt to help you understand and make the most of your benefits. It is important to understand that things can change between the eligibility check and the actual payment of the claim. For example, changes in insurance, lapse of time, new benefit years, claims that arrive to the payer after eligibility was checked. A quote of benefits is not a guarantee of benefits or payment. Charges not covered by your insurance company or benefit plan, as well as any co-payments, deductibles, and coinsurance amounts, are your responsibility.

Is My Treatment Covered by Insurance?

If your treatment is deemed to be elective (not medically necessary), it may not be covered by insurance. However, it is possible to pay for your out-of-pocket costs with funds from a medical spending account, like an FSA or HSA. We can also help you learn more about medical financing with CareCredit.

Payment for Premium Lenses

There are two separate payments for premium lenses. The fees for additional services such as measurements, calculations, markings, fitting and lens insertion are payable to CECA. The fee for the actual lens is payable to Stony Point Surgery Center. Please refer to the form indicating the costs payable to CECA and to SPSC. The amounts owed to SPSC will be payable the day of the procedure.

Please be aware that Medicare and other insurance companies consider these lenses and related services “optional” and will not cover these additional costs. This is a decision made by you, based on your lifestyle and your personal preference. If you have any questions, please call our office to discuss your options.

We do not offer a payment plan for services related to Premium Lenses. Payment in full is required 2 weeks prior to surgery. 

Please note: The fee for your premium lens services and lenses are separate from the charges for your cataract surgery. 

If you are having surgery at Medarva at Stony Point Surgery Center, you should be aware that SPSC is a joint venture between Richmond Eye and Ear Healthcare Alliance, Inc. and many of the physicians who practice at SPSC, including Dr. Michael, Dr. Iuorno, and Dr. Flowers. Though their ownership is a small percentage, it affords us at Commonwealth Eye Care Associates direct input into the quality of care that you, our patient, receive for your surgery.

Important Surgical Financial Information

You will have three separate charges and three separate insurance claims filed from your procedure. You will also have an office visit charge and charges for lens implant measurements. You may have deductible, copay or co-insurance amounts due after your insurance clears. This does NOT include premium lens charges.

Deductibles and co-insurances are due prior to surgery. We will contact your insurance company to determine these amounts. Please remember that this is an estimate based on the information received from your insurance company and is not a guarantee of payment. You are ultimately responsible for any allowed amounts not paid by your insurance.

The following guidelines help us provide timely and quality care to all patients.

Late Arrival

  • Patients are considered late if they have not arrived within 10 minutes of their scheduled appointment time.
  • Late arrivals may result in rescheduling or a reduced service depending on availability.

Cancellation

  • Appointments must be canceled or rescheduled at least 24 hours in advance.
  • Late cancellations may result in a $35 fee.

Missed Appointment

  • Missing an appointment without prior notice is considered a no-show.
  • No-shows may be charged a $35 fee.
  • If missed appointments persist this may affect future scheduling.

Compassionate Exceptions

  • We understand that life can be unpredictable.

Financing Options

Alphaeon Credit

With the Alphaeon Credit card*, you’ll find a wide array of flexible monthly payment options to help you receive the care your doctor recommends.

What Can I Finance?

Alphaeon offers financing for all treatments we offer, including:

*Subject to credit approval and availability. Purchase must be made using the ALPHAEON CREDIT Card Account. ALPHAEON CREDIT Card Accounts are offered by Comenity Capital Bank which determines qualifications for credit and promotion eligibility. Account must be in good standing at the time of purchase. At no time will the minimum payment due be less than $27.00. Minimum interest charge is $3.00 per credit plan. Standard variable APR of 32.99%, based on the Prime Rate. Comenity Capital Bank issues ALPHAEON CREDIT Card Accounts, which can be used with participating physicians.

Paying for Care with FSA, HSA, and HRA Accounts

Patients can use funds from medical spending accounts like FSA (Flexible Spending Account), HSA (Health Savings Account, and HRA (Health Reimbursement Account) to cover the cost of care. These funds are specifically designated for healthcare expenses, and are employer-sponsored and tax-deferred. Funds from medical spending accounts may not carry over to the next calendar year, so it’s wise to use them before you lose them.
